職責:
- 負責長者社區個案管理及照顧服務
- 制定長者個人照顧計劃、健康評估及認知訓練
- 協調及安排長者到戶服務
- 須輪班 (0900-1800 / 1100-2000)
工作要求:
- 持有社工文憑及社工註冊證明書
- 須具有兩年或以上長者服務、社區服務相關工作經驗
- 具良好溝通技巧、電腦文書工作
