Job Description
Responsibilities
- Provide counselling to support the psychosocial needs of District Health Centre (DHC) clients relating to their health status;
- Organize patient empowerment programs to individuals or in groups;
- Compile local community resources for clients’ reference when needed;
- Organize peer groups and volunteer services;
- Assist in promotion and publicity activities of the DHC;
- May need to take on flexible hours and occasional Saturday or Sunday duties.
Requirements
- Registration with the Social Workers Registration Board;
- 3 or more aggregate years of practice in social services;
- Experience in chronic disease management; handling cases with chronic diseases and conducting patient empowerment programs will be an advantage;
- Familiar with community support resources.
