Job Description

Key responsibilities:

  • Provide community rehabilitation and self-management programs, psycho-social support services, health education activities, and organize patients’ self-help groups

Requirements:

  • Registered Social Worker with diploma or degree in Social Work
  • Preferably with experience in services for people with chronic health conditions
  • Self-initiative, independent, flexible, good interpersonal skills
  • Good command of both Chinese and English

Work location: Kowloon West District

*Participants are normally required to work 44 hours per week. Participants may need to work in the evening, weekends and public holidays when necessary.
